The Bank acts as an insurance agent providing intermediary services to Towarzystwo Ubezpieczeń na Życie “WARTA” S.A., and is entered in the register of insurance agents kept by the Polish Financial Supervision Authority.Clients may verify the Bank’s entry in the register by submitting a relevant application to the Polish Financial Supervision Authority via the KNF website: https://rpu.knf.gov.pl/, after completing the form. The Bank acts for the benefit of multiple Insurers; Clients may ask the Bank for more details.

In the process of selling and servicing insurance policies, the Bank follows the rules set out in the “Recommendation for good practices in the Polish bancassurance market for protective insurance policies sold in connection with bank products,” as published by the Polish Bank Association in cooperation with the Polish Chamber of Insurance.

Questions & Complaints
Any complaints may be submitted by the policyholder, the insured or the person entitled under the insurance contract to Towarzystwo Ubezpieczeń na Życie "WARTA" S.A.:
- win writing to: skr. pocztowa 1013, 00-950 Warszawa 1;
- electronically, using the form available at www.warta.pl/reklamacje;
- by calling at 502 308 308 (call cost as per the operator's tariff);
- to each unit of TUnŻ “WARTA” S.A. in person – in writing or verbally for the record;
to the Bank:
- in writing – submitted in person to the Bank’s branch during branch office hours, or sent to the following address: Citi Handlowy, Departament Obsługi Reklamacji i Zapytań Klientów, ul. Goleszowska 6, 01-249 Warszawa;
- verbally – by phone or in person for the record during a visit to the Bank’s branch;
- electronically – to the Bank’s email address through Citibank® Online after logging in via the “Contact with the Bank” tab.
Benefit Payment

Upon occurrence of an insured event, the entitled person should submit an application for benefit payment directly to the Insurer. To that end, he or she may complete an online application available at www.warta.pl or a form made available by the advisor at the Bank’s premises or by the Insurer, or downloaded from www.warta.pl.

The entitled person must enclose with the application for benefit payment such documents as the Insurer may specify as necessary to determine the validity and amount of benefit.
