In the event of the insurance incident, the person entitled files for the insurance payout directly with the Insurer by filling out the form provided by the Relationship Manager or the Insurer in any of the branches or on www.warta.pl.

In the case of an event covered by the insurance coverage, the beneficiary submits an application for the benefit payment directly to the Insurer. To this end, they shall submit a form made available by the employee of the Bank’s Branch or by the Insurer — at any branch or at www.warta.pl.
The date of receipt of the application by the Insurer shall be deemed as the date of submission of the benefit payment application.
